Kick It Official Team Roster Form

Kick It Official Team Roster - All teams participating in a Kick It 3v3 tournament must fill out this form and hand in to a Kick It staff member prior to their first game.  Teams that fail to hand this form in will be disqualified from the tournament.  No refunds will be issued to teams that are disqualified from a tournament. 

Please note:  This form requires the signature of the parent or legal guardian of anyone under the age of 18 so should be filled out prior to check-in.
